Add and Drop/Withdrawal Request Forms
If you
need to add or drop one or more courses after the late registration period, please follow the
steps below:
1. Be aware of the add/drop dates for the current semester in the Academic Calendar.
2. Download and print the form you need below.
3. Carefully read the form in its entirety and fill in all the requested information, making sure to check all necessary boxes.
4. Take the form to the instructor of each course to obtain a signature of approval.
5. After the
form is completed and signed, bring it to the Certification & Advising Office
in 501 Carlisle Hall for processing.
An appointment is
not needed; you may come during the hours of 8:30 a.m. - 5:00 p.m., Monday -
Friday.
NOTE: Secondary and All-Level students should see
their major advisor to add or drop courses.

This form
should be used by undergraduate students requesting to drop one or more courses
after the late registration period
and no later than the last drop date.
NOTE: Students needing to completely withdraw from the university before the end
of late
registration must still submit this form. Add/drop deadlines for the current semester can be
viewed in the
Academic Calendar.
