Field Experience Approval Form

Secondary and All-Level Art certification students must have their content advisor complete a Field Experience
Approval Form before they can be cleared for Field Based Courses courses.

The form can be printed off this website or you can request one from an Education advisor. You will
then take the form to your content advisor, who will fill in how many content courses you have remaining,
if any, and sign the form. You can then return the form to the Education advising office.

The purpose of this form is to let the Education advisors know that you will be through with your content
courses before Internship. If you have any questions, feel free to contact an advisor.
