Registration Clearance Request Form

Purpose of this request form:
  -EC-6 and Mid-Level students will have their Academic Department holds removed
  -Education courses require department consent; you may request to be added to the permission lists.

Students who have not had an advising appointment in approximately  a year (March 2011 and before)
may not be eligible to submit this form and must call 817-272-2956 to schedule an advising appointment.

EC-6 and Mid-Level students planning to enter the program this fall must attend an advising orientation.  Therefore,
you will not need to request clearance for education courses below.  You obtain clearance through the orientation.
DO NOT select any of the courses below unless it's MATH 4350 for Mid-level students or BEEP 3381 for Bilingual/ESL students for Fall.

*For Secondary English certification only
**History/Social Studies majors will take EDUC 4343 instead of EDUC 4342 during Field Based Experience.
