Steps for Requesting
Accommodations at UT Arlington |
Here are the steps for requesting
accommodations at UT Arlington for a disability.
- Provide a copy of the testing, medical records, and/or a letter
from a physician or appropriate clinician that indicates your
disability. See various handouts with specific documentation requirements
by specific disabilities to determine what documentation is appropriate
for your disability(s). Submit this information to the Office
for Students with Disabilities, University Hall room 102.
- Your documentation will go before the Documentation Review
Committee (DRC) where your eligibility for services will be determined.
A counselor will contact you after your documentation has been
reviewed. The committee meets every week with the exception of
university closures/holidays. Documentation must be handed in
to the OSD before 5pm on Thursday to be considered by the DRC
that week. Anything received after 5 p.m. will be reviewed the
following week. Please allow seven business days for a counselor
to contact you after your documentation has been reviewed to schedule
an appointment if necessary.
- Once you have been registered in the OSD and have met with
a counselor, you must complete the "Request
for Accommodation Form", and leave it with the receptionist.
- Letters will be generated based on the form and based on those
accommodations agreed upon with the OSD counselor. The letter
will outline specific instructions for your instructors with the
appropriate accommodations. You must come back to our office (allow
at least 48 hours) after you leave the request form to pick up
the letters.
- You will be asked by the receptionist to sign and date the
"Accommodation Letter Pickup Sheet". The letters should
be personally delivered by you to each of your instructors. It
is best to do this during his or her office hours. Accommodations
do not become effective until the instructor receives the request
by letter. Students deliver their letters personally to preserve
confidentiality and to create an opportunity for discussion between
the student and the instructor.
- Repeat steps 3-5 at the beginning of every semester you are
enrolled at UT Arlington.

General Documentation
Guidelines for Students with Disabilities at UT Arlington |
In order to fully evaluate requests for accommodations
or auxiliary aids and to determine eligibility for services, the
Office for Students with Disabilities (OSD) needs documentation
of your disability. The documentation you provide should include
an evaluation by an appropriate professional that makes evident
the current impact of the disability as it relates to the accommodation(s)
requested and include a description of any and all functional limitations.
Those accommodations prescribed by the OSD are provided so that
students have equal access to activities and programs at UT Arlington. Professionals
conducting assessments and rendering diagnoses must be qualified
to do so. Such documentation should be on letterhead and contain
the professional's signature and license number. Documentation must
be current.
The general guidelines listed below are developed to assist you
in working with your treating/diagnosing professional(s) to prepare
the information needed to evaluate your request(s). If, after reading
these guidelines and reviewing disability specific information provided
below, you have any questions, feel free to call the office at 817-272-3364.
Documentation should include the following information:
- Current functional impact of the condition(s)
The current relevant functional impacts on physical (mobility,
dexterity, endurance, etc.), perceptual, cognitive (attention,
distractibility, communication, etc), and behavioral abilities
should be described as a clinical narrative and/or through the
provision of specific results from the diagnostic procedures/assessment.
- Treatments, medications, accommodations/auxiliary aids, services
currently prescribed or in use.
Provide a description of treatments, medications, accommodations/auxiliary
aids and/or services currently in use and their estimated effectiveness
in minimizing the impact of the condition(s). Include any significant
side effects that may impact physical, perceptual, behavioral
or cognitive performance. If you feel that any additional accommodations/auxiliary
aids are warranted, please list them along with a clear rationale
and related functional limitations. Any accommodations/auxiliary
aids will be taken into consideration, but not automatically implemented.
- The expected progression or stability of disability over time
If possible, provide a description of the expected change in the
functional impact of the condition(s) over time. If the condition
is variable, describe the known triggers that may exacerbate the
condition.
- A diagnostic statement identifying the disability
When appropriate, include International Classification of Diseases
(ICD) or Diagnostic Statistical Manual (DSM) codes, the date of
the most recent evaluation, or the dates of evaluations performed
by referring professionals. If the most recent evaluation was
not a full evaluation, indicate when the last full evaluation
was conducted.
