DOCUMENTATION REQUIREMENTS

Accommodation Forms

IMPORTANT! The forms are provided for your convenience. Please complete the form and either fax it to (817) 272-1447, or mail it to Box 19510; University Hall, Room 102; Arlington, TX 76019-0355, or hand deliver it to the OSD office located in Room 102, University Hall.

All Accommodation Requests for tests must be made at least 7 business days prior to the test date. All other accommodation requests deadlines will be handled on a case by case basis.

DO NOT EMAIL THE FORMS. After our office recieves the form it will be reviewed by the appropriate OSD staff and if approved, letters will be created for the classes you list. If you are requesting accommodations for the first time, or if requesting different accommodations than last semester, please call our office at (817) 272-3364.
