Instructions to Student:
We anticipate the number of applications to exceed the session limits. It is therefore extremely important that you provide a complete application packet. We are continuing to accept applications so please follow these instructions to submit your application as soon as possible:

• Have your mathematics or science teacher complete the evaluation form and return it to you in a sealed envelope with the teacher’s signature across the seal.

• Obtain a copy of your transcript or cumulative folder for this year and last year from your school principal or counselor. (A recent report card will not be accepted in lieu of a transcript).
