Instructions to Student:
We anticipate the number of applications to exceed the session limits. It is therefore extremely important that you provide a complete application packet by the submission deadline of March 31st. Please follow these instructions:

• Complete the application form (please type or print).

  You will need Adobe Acrobat reader (available for free from www.adobe.com) to view and print these forms.
  
• Have your mathematics or science teacher complete the evaluation form and return it to you in a sealed envelope with the teacher's signature across the seal.

• Obtain a copy of your transcript or cumulative folder from your school principal or counselor. (A recent report card will not be accepted in lieu of a transcript.)

The schedules for the 2008 Summer Camps are:

• Mail these three items as a single application packet.  The packet must be postmarked by March 31.
