The University of Texas System contracts with
the Mega Life and Health Insurance Company to provide a Preferred Provider
Organization (PPO) insurance plan to UTA students. A few basic benefits
of this plan are outlined below. Please access the online brochure for
complete coverage information.
Treatment at the Health Center
Some procedures performed at the Health Center
are covered at 100%. This coverage includes laboratory, pharmacy (co-pays
are $10/generic, $15/brand name), x-rays, most gynecology (click here
for details), and mental health visits. Preventive procedures such as
immunizations are not covered. Pre-existing conditions and the annual
deductible do not apply to visits at the Health Center.
Treatment at another Facility
When it is necessary to have treatment at
a facility other than the UTA Health Center, an annual deductible of $300
(for each plan member) must be met before the insurance company will consider
reimbursement (including pharmacy charges). Once this deductible is met,
the insurance company will reimburse you at a rate of either 80% (preferred
providers) or 60% (non-preferred providers) for covered procedures.
The insurance company will not cover pre-existing conditions until you
have been insured for one full year without a break in coverage. If you
have been covered by another insurance within the last 63 days, the pre-existing
clause does not apply to you. Please inform the insurance company of your
previous coverage.

Optional Dental and Vision Plans for
Student Insurance
As always, if you have any questions concerning your insurance plan, you
may contact the Health Center at 817-272-2771.
If you have either HMO Blue or UT Select, the vision and dental plans
mentioned in this section do not apply to your insurance. If you are interested
in purchasing dental or vision coverage, please contact the UTA Human
Resources Department at 817-272-5554.

UTA offers their Teaching Assistants (GTAs and TAs), Research Assistants
(GRAs and RAs) and Faculty/Staff UT Select
as a health insurance provider. See below for an explanation of how this
plan is utilized at the Health Center.

UT Select is a PPO plan that covers most procedures
performed at the Health Center. Since we are providers for UT Select,
procedures performed here are considered "in-network" and will
be claimed to your insurance. If your covered family members are UTA students
or faculty/staff, we provide services to them also, but do not claim your
insurance for treatment.

As a GTA, GRA, TA, or RA covered by UT Select
(bring your insurance card) you may utilize the Health Center
without paying a co-pay for your general medical visit as long as you
are an enrolled student in the current semester and have not waived
your student fees.

Faculty/Staff covered by UT Select may utilize
the Health Center on an urgent care basis, but will be responsible to
pay the co-pays for general medical visits and prescriptions. Please
