Health Requirements

Meningitis requirement

Incoming college students and students returning after an absence of at least one fall or spring semester who are under the age of 22 must submit documentation of immunization against bacterial meningitis. Students who fail to meet this requirement by the deadline will be dropped from courses. New international students are highly encouraged to receive their vaccination in their home country in order to meet the deadline for providing documentation for proof of the vaccination.

The law mandates that the following students be vaccinated against the bacterial meningitis:

  • First-time college students
  • Students transferring from another institution
  • Students who are re-enrolling following an absence of at least one fall or spring semester
  • Students previously or currently enrolled exclusively in UT Arlington online courses who enroll in a UT Arlington in-seat course in subsequent sessions
  • Individuals under age 22 who plan to audit a course


Students may use the UT Arlington Bacterial Meningitis Immunization Record form as proof of vaccination. A portion of this form must be completed and signed/stamped by a healthcare provider or pharmacy.

Students may submit this form or their vaccination record directly to UT Arlington online or by fax (817-272-3223).

tuberculosis (TB) requirement

To meet university requirements and have health requirement registration holds removed, TB screening must be completed. We accept both T-Spot and Quantiferon Gold blood tests if done within the last 12 months in the United States. PPD (Mantoux) skin tests are not accepted. Students who have not had TB screening completed in the United States within the last 12 months will need to get tested in the health center.

TB screening is available without an appointment beginning January 2. Students may walk-in for TB screening Monday through Friday, 8:30 AM – 12:00 PM and 1:00 PM – 4:00 PM. TB screening is not available 12:00 PM - 1:00 PM.

An appointment is required for TB screening conducted January 13 – 17, 2020.  To schedule an appointment, please call 817-272-2771 and speak with a staff member. Students cannot leave leave a message to schedule an appointment. 

Please eat food and drink water before arriving at the health center for TB screening.

Please click on the following link to access the TB Outreach Packet that must be completed before receiving TB screening. Log in using your Net ID and Password (same username and password used when checking UTA email), click on Forms, and then complete the TB Outreach Packet.

If you have already completed TB screening in the United States within the last 12 months, please submit your documentation to the health center in person or by fax to 817-272-3829. To be accepted, your documentation needs to be signed and stamped by the U.S. clinic you received the test from, and must include the lab report.

The cost of TB screening is $69. If a chest x-ray is indicated, there is an additional charge of $59.  The UT System student health insurance plan (UT SHIP) will only cover TB screening received on or after the coverage plan start date. (Click on the Insurance tab on the top of this page for more information on dates of coverage.) PPD (Mantoux) skin tests are not accepted by the health center.

If you are planning to do your TB screening at a U.S. clinic other than UTA, please have the following form filled out by the health care provider doing your screening:

Fax the completed form to Health Services at 817-272-3829.  Once we receive your completed form, your holds will be removed before the end of the next business day.
