PURPOSE: Accomplishes initial intake interviews with patients or their families; explains clinic or hospital procedures; answers patients' questions concerning clinic or hospital routines; sets up schedules for clinic visits as appropriate. Works under general supervision to provide experienced paraprofessional social work services for clinic or hospital patients.

DUTIES AND RESPONSIBILITIES: Identifies immediate social, financial, educational, or other family problems, referring those requiring professional attention to other members of social work staff; assists in making patient referrals, following up to determine whether patients' needs have been resolved. Provides initial counseling to patients with respect to certain selected routine problem, areas, such as budget management, school absence, parent-child problems, health maintenance, and use of other educational or welfare resources. Follows-up patients through home visits, correspondence, or telephone concerning missed clinic appointments, particularly where there is high medical risk involved; assists patients in rescheduling and keeping new appointments. Arranges transportation for patients to special clinics when necessary. Acts as patient advocate in emergency situations. Completes patient forms and records; prepares chronological notes on each assigned patient; completes daily and weekly activity and travel reports; maintains necessary records and files. May provide working supervision to lower level employees, may assign work, follow up to ensure proper completions, and advise as to resolution of work problems. Performs other related duties as assigned.

REQUIRED EDUCATION-OTHER: Graduation from high school or GED equivalent.

REQUIRED EXPERIENCE / COMPETENCIES-OTHER: REQUIRED EXPERIENCES/COMPETENCIES: One (1) year relevant work experience to include at least six months experience as Case Aide or equivalent. OTHER REQUIREMENTS: Proficiency in all phases of duties performed. Work requires accuracy in locating files, recording interview information, and in adhering to clinic or hospital policies and procedures. Work requires ability to understand and follow established procedures or other guidelines of some complexity in performance of work tasks, which involve making of frequent minor decisions or occasional decisions of some importance. Work requires use of some initiative, independent judgment, or resourcefulness. Work requires devotion of 50% or more of time to routine contacts while interviewing patients or their families or in representing the clinic at various community affairs. Working conditions are considered to be fair and involve more than average (25-50%) exposure to one or more disagreeable elements, such as need to travel to patient's homes, working under crowded or noisy conditions, etc. Specific job requirements or physical location of positions allocated to this classification render the position security sensitive and thereby subject to the provisions of Section 51.215 Texas Education Code.
