PURPOSE: Interviews patients and sometimes members of their families to determine general problems and needs. Works under constant supervision to provide medical and psychiatric social casework services of limited scope for hospital or clinic patients.
DUTIES AND RESPONSIBILITIES: Counsels patients with respect to clearly-defined medical, surgical, or other programs. Prepares and maintains chronological notes, clinic charts, or case histories for each patient; summarizes and records all pertinent information on designated forms and other documents which become part of patients' permanent files. Confers with supervisor or other professional social workers of higher grade for guidance as to best means of assisting patients to resolve general problems, such as referrals to community social service agencies, health maintenance, employment, financial assistance, physical rehabilitation, etc. Makes clinic appointments or referrals to other clinics and social service agencies as necessary. Maintains patient follow-up by home, hospital visits, or telephone to determine whether patients' needs have been met and problems resolved. Prepares daily, weekly, or monthly statistical or activity reports as required. Participates in staff conferences; assists in conducting community education programs as required. May provide working supervision to case aides of lower grade; may assign work, follow up to ensure proper completion, and advise with respect to resolution of work problems. Performs other duties as assigned.
REPORTING RELATIONSHIPS:
REQUIRED EDUCATION-OTHER: Bachelor's degree in social work, sociology, or psychology or related field.
