Completion of this form is the first step in the process. It will be sent to the Information Resources Coordinator who will acknowledge receipt. After thorough review, you will be informed of all decisions regarding the retention or disposition of materials. If you are not satisfied with the decision, an appeal may be made to the Dean of the Library.
