RN to BSN

FALL 2009
-- Returning BSN Students are expected to submit written documentation
of the following clinical requirements valid for the entire academic
semester (through
December, 2009):
Documentation of Immunization or Immunity Titer for:
-
Negative PPD
with results reported as negative and size in mms induration (TB
Skin Test required ANNUALLY) OR Negative Chest X-ray Report within
one year of admission to UTA SON for Positive PPD and results in mms
induration with follow-up treatment according to physician.

MMR (Measles, Mumps and Rubella) 2 immunizations or history of
exposure
(CDC guidelines recommend 2 vaccines if born after 1957)

Chicken Pox --
2 immunizations or an immune titer
-
Tetanus Booster
within the last 10 years (must be a Tdap - Tetanus, Diptheria,
Pertussis)
-
Completion of
DFW Hospital Council Standard
Student Clinical Orientation and 100% on post-test
(during JR I or SR I Clinical Orientation) to be
completed annually

Students who have not
been cleared for clinical requirements by
August 21st will not be permitted to
participate in clinical until clinical requirements are met.
If
your CPR Certification or clinical requirements expire
DURING the Fall 2009 Semester, you are expected to renew prior
to the start of the semester.
