Social Security Request
Please read these instructions then click the link at the end to print the form using Adobe Acrobat; note that the form is fillable. Students will need to bring the completed form to the Office of International Education. In 7-10 business days, an International Education Advisor will sign the On Campus Employment Verification Form and e-mail the student when the form is ready. The student may then collect the form at the front desk of the Office of International Education.
This form is to be used when applying for a Social Security Number based on an on campus employment offer only. Any questions about the completion of this form or the requirements for showing proof of on campus employment may be directed to the Office of International Education by calling (817) 272-2355 during business hours.
If you reside outside the DFW metroplex you may refer to the mail-in/mail-out process.
