Activity Report for the previous academic year--should be two-pages maximum and summarize all activities in the areas of teaching, research, and service for the previous two years.

1-2 paragraphs summarizing activities for the previous academic year.

1-2 paragraphs summarizing your accomplishments (for posting on your web site--not necessary for those who have their own web site).

Updated Vita (no personal information).

Outside Employment Form: All employees currently engaged in outside employment must complete the Outside Employment form found at: http://policy.uta.edu/forms/hr/3-19.pdf. Please complete the form, print it out, and provide it to your Department Chair (who will save a copy before sending it to the Dean for signature and submission to the Office of the Provost. An employee not engaged in outside employment does not need to complete the form.

Faculty-authored textbooks approval. Any faculty member who is requiring purchase of a book for which they are the author must receive approval. The appropriate form may be found at: http://www.uta.edu/provost/facultysupport/forms/facauthor.pdf. This form should be completed and submitted to the Department Chair for submission to the Dean and to the Provost.
