The following files provide samples of the questions used in most Student Feedback Surveys.
Note that the visual format of the actual online surveys will differ from that of the PDF files provided by the links below.
Form F2F
This is the survey’s base form. It is used by students enrolled in the majority of organized courses at UT Arlington.
Form DE
This form is a variant of Form F2F. It has been adapted for use by students in distance education classes.
Form LAB
This form is a variant of Form F2F. It has been adapted for use by students in laboratory courses.
In addition to these three most common forms, there are minor variants that have been commissioned for distance ed lab courses, courses in the College of Nursing, and courses in the Department of Music.
