Bilingual Service Coordinator 1
Conducts telephonic assessments to members to provide health coaching, structured assessments, and may develop appropriate care plans. Responsible for collaborating with healthcare providers and members to optimize member benefits and to promote effective use of resources. Provides guidance to members regarding barriers to managing health conditions. Assists members to change behaviors and to locate and access interpersonal, family, and community resources that will make it easier to manage their health.

Must be bilingual-English/Spanish. Must have current licensure as a Licensed Vocational Nurse (LVN) or a Bachelors of Social Work (BSW). Must have one year of case management experience or customer service experience in a hospital, doctors office, medical insurance, home health or managed care environment. Must have knowledge of managed care, medical management and related policies and procedures. Intermediate skill level with Microsoft Office products to include Word, Excel and Outlook. Excellent organizational, written, and oral communication skills.
