Implement and administer policies, programs and activities. Responsible for performing psychosocial assessments on families ability to meet their child’s healthcare needs. Ensure the preparation and maintenance of required documentation per accreditation standards and CMC’s policy. Requires in-depth professional knowledge and practical/applied expertise in own discipline and basic knowledge of related disciplines within the broader professional field. Has knowledge of best practices and how own area integrates with others; demonstrates awareness of the industry, including regulatory, evolving customer demands, and the factors that differentiate the organization in the market.

Essential Duties and Responsibilities
Perform biopsychosocial screening and assessment of the families ability to meet their children’s healthcare needs by applying specialized knowledge and advance practice skills in assessment treatment, diagnosis of mental illness, planning, implementation and evaluation.
Facilitate parent/child adjustment to illness and hospitalization.
Coordinate anticipatory discharge to ensure that patient’s home – care is prearranged prior to leaving the hospital.
Coordinate and direct multidisciplinary patient care conferences focusing both on medical and biopsychosocial needs.
Two to three years of previous Social Work experience required.
Four to five years of related experience preferred.
Graduate or professional work or advanced degree; or equivalent experience
LICENSES and CERTIFICATIONS
State of Texas LMSW license required.
State of Texas LCSW license preferred.
