Refugee Mental Health Lead Case Manager

Job Summary
The Mental Health Lead Case Manager position aims at supporting refugee clients who have mental illnesses to access services and link them to appropriate services. The Lead Case Manager advocates for clients in a safe and culturally appropriate manner. The Lead Case Manager is also responsible for conducting comprehensive case management intakes for refugee clients/families and develop individual service plans (ISP). Finally, the Lead Case Manager will be responsible for training and supervising Cultural Ambassadors.

Job Duties

Main Responsibilities:

o Conducting intake and needs assessment for refugee clients with mental health needs
o Determining enrollment eligibility and appropriate services for refugees with mental illnesses
o Developing, implementing, monitoring, and evaluating individualized service plans with each client; linking each client with appropriate community mental health supports to ensure their progress towards the individualized service plan goals.
o Maintaining a good working relationship with clients, making home visits as needed
o Developing and maintaining progress notes, and other documentation up to date
o Reviewing and updating individual treatment plans on quarterly basis
o Assisting Cultural Ambassadors with their paperwork documentation
o Compiling case management data, completing case reports and providing feedback to PRE management
o Coordinating community support groups activities with Cultural Ambassadors
o Monitoring community support groups’ progress and giving suggestions/recommendations to PRE management
o Coordinating clients transportation with Cultural Ambassadors
o Supervising Cultural Ambassadors for case management skills
o Conduct individual weekly supervision meetings with Cultural Ambassadors
o Reviewing clients files for both case management and community support groups
o Advocating for clients with service providers, agencies, and communities as needed
o Conducting program outreach in the refugee communities
o Developing and nurturing a good working relationship between PRE and refugee communities in the Fort Worth area

Other Duties:

o Developing training curriculums and resources on community and refugee mental health
o Overseeing the development and delivery of periodic trainings for Cultural Ambassadors and other PRE full-time and part-time staff
o Developing and delivering presentations for community partners on the refugee mental health needs as needed
o Researching and developing program’s mental health assessment tools and instruments
o Researching and analyzing client population trends and emerging mental health needs
o Perform any other assigned duty relevant to this position

Qualifications:

o Have a Bachelor Degree of Social Work or a college Degree in related field
o Experience in case management and specifically working with refugees preferred
o Ability to deal with clients from various cultural and linguistic background
o Ability to work with clients with mental illness and in some cases with clients in crisis
o Demonstrate knowledge of refugee, asylee, and immigrants’ status
o Supervisory experience preferred
o Ability to make presentation in community support groups or in various providers’ meetings-conferences
o Ability to develop curriculum and educational material for refugees
o Willingness to conduct home visits as needed
o Willingness to transport clients using personal vehicle when necessary
o Must have excellent written and oral communications skills in English
o Must have reliable transportation, a Texas DL, and auto liability insurance

Work hours and Job site:
o PRE regular office hours are 8:00 am to 5:00 pm, Monday through Friday. This position may require some after hour work depending on clients’ needs.
o This position requires some traveling between PRE’s office and clients’ locations
