Only needed if traveling outside of DFW Metroplex. ALL THREE DOCUMENTS MUST BE COMPLETED AND SUBMITTED AT THE SAME TIME. If you have questions on how to fill out these University required documents, please contact Student Governance & Organization at least seven (7) days prior to travel.
