How to Refer a Policy Violation

Any person may refer a policy violation against any active or prospective UT Arlington student for alleged violations of University policy, occurring on or off campus.

To Refer a Policy Violation You Must;

Submit a narrative of the incident to the Office of Student Judicial Affairs, located in E.H. Hereford University Center, lower level, B160, Box 19355. This narrative can also be submitted via email at judicial@uta.edu or fax to (817) 272-5221.

A Referral Must Include:

• Referrer's name, title and contact information
• Nature and date of the alleged violation
• Name of person(s) responsible for the alleged violations(s)
• Detailed statement of the incident

Referrals may be originated by students, faculty, staff, University departments and individuals external to the University.

After a Referral Is Filed

The Office of Student Judicial Affairs is responsible for investigating all allegations of policy violations received. If an allegation is found to contain a policy violation, the alleged person(s) involved will be sent a summons that requires them to schedule an appointment.
