Formal Complaint Form

The Office of Student Governance & Organizations has an open door policy to discuss informal complaints.  Informal complaints regarding policies, processes, and programs should be discussed with the staff member responsible for that area.  (For example, complaints about a program, such as one hosted by the UTA Ambassadors, should be discussed with the Advisor who works with that organization.  Complaints about a posting policy should be discussed with the Director.)  In addition, we welcome comments and suggestions for improvement.

 

Formal complaints regarding policies, processes, or staff must be submitted in writing or by email.  Complaints regarding staff should be submitted to the Director of Student Governance & Organizations.  Complaints should be submitted within 10 class days.  Within 72 hours of receiving the written complaint, the Office of Student Governance & Organizations will respond regarding the complaint process:

  • For policy and process complaints:

    • Staff will review the issue with the Director of Student Governance & Organizations.

    • The complainant will be notified in writing of the result of that meeting and any further decisions made regarding the issue within 10 class days of the receipt of the complaint.

    • If the complainant wishes to appeal the decision, he/she will contact the Assistant Vice President for Student Affairs over Student Governance & Organizations.

    • If the complainant is not satisfied with the resolution, an appeal may be made in writing to the Vice President for Student Affairs.

    • Any further action will be determined by the Vice President for Student Affairs.

    • For complaints regarding staff:

      • The Director of Student Governance & Organizations will strive to resolve the issue with the staff person involved.

      • The complainant will be notified in writing of any decisions or changes made within 10 class days of the receipt of the complaint.

      • If the complainant wishes to appeal the decision, he/she will contact the Assistant Vice President for Student Affairs over Student Governance & Organization.

      • If the complainant is not satisfied with the resolution, an appeal may be made in writing to the Vice President for Student Affairs.

Any further action will be determined by the Vice President for Student Affairs
