Scheduled Appointment - Telehealth Guide

Please follow the Telehealth Guide below after you have a scheduled visit. To schedule a visit, call CAPS at 817-272-3671. 

Before your zoom telehealth visit:

Consider technology and privacy requirements to minimize disruptions and maximize confidentiality. Please make sure you have the Zoom app downloaded to your device and a free Zoom account established.

First-time telehealth visit or instructed by CAPS: 

Log into health services web portal 

Complete Health Services forms (A-H) immediately after your visit is scheduled. Ensure Form-F (Informed Consent) is fully completed. You may then skip Step 2 in step-by-step guide below.

Crisis, First-time telehealth visit, or instructed by CAPS:

30 Minutes Prior to Scheduled Visit: begin paperwork via web portal 

Complete Form-F (Informed Consent), questionnaire (SDS) and survey (CCAPS-62). All paperwork must be completed (Steps 1-4) prior to start of visit.

STEP-BY-STEP GUIDE:

STEP 1: GETTING STARTED

Log into web portal with UTA login and password

Confirm your date of birth and "Proceed".

STEP 2: FORMS

Left-hand side: FORMS.

Scroll down, complete FORM F - CAPS CONSENT, TELEHEALTH, AND EMERGENCY FORMS, highlighted in blue.

STEP 3: APPOINTMENTS

Left-hand side: APPOINTMENTS ->Appointment Check In.

Confirm your physical location (must be in Texas unless in crisis).

Click "OK" next to “Check-In to Online Appointment”- > DO NOT CLICK: If given “Click here to join the meeting” option.

Click “Return to Appointments”.

Click on “Complete Questionnaire”. Complete all applicable questions, click “Submit Final”.

STEP 4: SURVEY FORMS

Left-hand side: SURVEY FORMS

Select CCAPS-62 (unless CCAPS-34 instructed). Complete all questions, click “Finish”.

STEP 5: JOIN MEETING

Left-hand side: APPOINTMENTS

Select "Join Meeting" to enter the Zoom waiting room. Please wait here until the counselor starts the session.

If you have any questions or difficulty with this process, please call CAPS at 817-272-3671 for assistance. In case of a loss of connection during the video session, your counselor will call you at the phone number you provided.
