Schedule an appointment

Visits to UTA Health Services are available by appointment only. Log onto the secure patient portal https://mavcare.uta.edu or call 817-272-2771 to schedule an appointment. The length of your appointment will vary based on the services provided. A routine visit with a physician or nurse practitioner averages 20 minutes in length. This does not include additional time needed for lab testing, x-rays, or filling a prescription in the pharmacy. Please consider this when scheduling your appointment, especially if you are trying to schedule an appointment before or between classes.

MISSED APPOINTMENT, LATE CANCELLATION

If you need to cancel or reschedule an appointment, please call 817-272-2771 at least four hours prior to your appointment time.

If you fail to cancel or reschedule, a no-show or missed appointment charge of $20 will be charged to your account. 

Eligibility

Undergraduate and graduate students that have the Medical Services Fee included in their tuition and fees are eligible for services in the health center.

Privacy and Confidentiality

UTA Health Services is committed to providing excellent health care, which includes maintaining the privacy and confidentiality of your health information. Health Services cannot disclose your health information to an individual, organization, or facility (including other campus departments, faculty, or staff) unless you give permission for us to do so. Our Notice of Privacy Practices describes how your health information may be used and disclosed and how you can get access to your health information.

Patient Rights and Responsibilities

As a patient of UTA Health Services, you have the right:

  • To be treated with respect, consideration, and dignity.
  • To be provided appropriate privacy at check-in and while being seen.
  • To know your provider’s name and credentials.
  • To be given complete information regarding your diagnosis, treatment, and prognosis when medically advisable and known. Such information may also be provided to another person under the following conditions: during a medical emergency, when it is medically inadvisable for you to receive the information about your prognosis, when the person has legal authorization, or when you give authorization for the person to receive the information.
  • To be given the opportunity to participate in decisions involving your health care.
  • To change providers if other qualified providers are available.
  • To decline evaluation and/or treatment by a health care professional in training.
  • To be informed of the right to refuse to participate in any research at UTA Health Services.
  • To provide feedback, including complaints or grievances. Email healthservices@uta.edu or complete a patient feedback form online.

As a patient of UTA Health Services, you have the responsibility:

  • To provide valid identification at each visit.
  • To be on time and if unable to keep your appointment, to cancel immediately so another patient can be scheduled.
  • To be respectful toward all providers and staff, as well as other patients and visitors.
  • To provide complete and accurate information to the best of your ability about your health, any medications, including over-the-counter products and dietary supplements and any allergies or sensitivities.
  • To follow the agreed-upon treatment plan prescribed by your provider and participate in your care.
  • To ask for clarification if information or instructions are not understood.
  • To provide a responsible adult to transport you home from the facility and remain with you as directed by your provider.
  • To accept personal financial responsibility for any charges not covered by the Medical Services Fee and/or the student health insurance plan.
