The Signed by date is typically the Census Date for the session. Both the student and instructor need to have signed the form before the deadline.
The Approval by date only applies to Contracts and not Designation forms. By this deadline, the instructor must sign off that the work was done satisfactorily as stipulated by the contract.

NOTE: This system is for credit contracts only. If you require an Honors Designation Form, download it at the link below. Remember, the Honors Designation Form is intended for use with very specific courses: an Independent Study or Conference Course used for directed study, non-Honors Study Abroad programs, Graduate courses, Honors College Undergraduate Research Fellowships (URFs), and the Engineering Senior Design course. All other courses use the Credit Contract system.
