Enrollment Verifications

This is for enrollment verifications, letters of intent, and letters of non-attendance.

If you have a FORM that must be filled out by the Registrar's office, you can submit it as an attachment at the bottom of this form.

YOU MUST SUBMIT YOUR GOVERNMENT ISSUED (or UTA issued) PHOTO ID, ALONG WITH YOUR HANDWRITTEN SIGNATURE AT THE BOTTOM OF THIS FORM. YOUR REQUEST WILL NOT BE PROCESSED WITHOUT THEM.

**Requests are generally processed within 7-10 business days.
