Students interested in attending the Alternative Breaks Spring trip to Houston, TX and Galveston, TX on March 12th-17th, 2018 must complete both this application and the online Alternative Breaks Agreement (the link for the second form will be given to you upon completion of your application).

Participants are chosen based upon a selection process. Completing these forms does not guarantee that an applicant will be selected as a participant on this trip.

The total cost of the Alternative Spring Break trip for participants is $200.00 - this amount will cover all lodging and transportation costs, and the majority of meals for the entire trip.

- Deposit of $100.00 due by Friday February 2, 2018.
- Any Remaining Balance is due by Tuesday February 27, 2018.

NO PAYMENTS WILL BE ACCEPTED UNTIL A PARTICIPANT HAS BEEN SELECTED.

If you are selected as a participant, you will be notified via the e-mail address you provide us, and will then be required to pay a $100.00 deposit by February 2, 2018. Failure to make a deposit of at least that amount by 2/2/18 will result in the loss of your spot on the trip.

Any remaining balance of your trip cost is due by February 27, 2018. Failure to be paid in full by this date will result in the forfeiture of your spot without refund, and an alternate participant will be given the option to attend in your place.

All payments (debit/credit card only) are to be made at the Follett Student Leadership Center located on the main level of the University Center (Suite 180S). Payments are accepted on Tuesdays & Fridays between 8-11am and 2-5pm.
