Hazard Reporting

Procedures
  1. Administrators, faculty, staff and students are encouraged to identify and report actual or potential hazards to the Environmental Health & Safety Office. During normal business hours you can contact Environmental Health & Safety Office at ext. 2185. After hours contact the UTA Police Department at ext. 23381.

  2. Once a safety hazard is identified, the Environmental Health & Safety Office will bring it to the attention of the agency/department that has the responsibility for eliminating or minimizing the problem. That agency/department shall provide a priority for allocating resources to correct the identified hazards. The Environmental Health & Safety Office will monitor to ensure the corrective actions are adequate.
