Hazlewood Exemption Act Important Information

  • All Hazlewood users must apply every academic year starting with Fall.  The exemption will only be applied to the terms selected on the application.
  • Beginning with the fall 2019 term all initial applications and supporting documents must be submitted online through the appropriate link below.  
  • Continued Enrollment applications are submitted online through a separate link below.
  • You must be registered for classes, tuition and fees must be present on your UTA account, and grades must have posted for the previous semester in order for your Hazlewood Exemption to apply. If your application is not received by payment deadlines, you will be responsible for making payment arrangements with Student Accounts and will be reimbursed when Hazlewood posts. 
  • Hazlewood does NOT pay the Student Service fee.  You are responsible for payment of that fee before payment due date.
  • All students receiving the Hazlewood exemption must meet the school's Satisfactory Academic Progress (SAP) requirements.  Senate Bill 1210 - Sec. 54.2001 states that a student may receive the tuition exemption only if:

    • As a graduate or undergraduate student, maintains a grade point average (GPA) that satisfies the institution's GPA requirements for making satisfactory academic progress (SAP) toward a degree or certificate in accordance with the institution's policy regarding eligibility for financial aid.
    • As an undergraduate student, has not completed as of the beginning of the term, a number of semester credit hours that is considered to be excessive as outlined in Texas Education Code 54.014, unless permitted to complete those hours by the institution on a showing of good cause.  View UTA Excessive Hours rules.

Students who fail to meet SAP standards and lose the Hazlewood exemption may appeal. The appeal form and applicable documentation should be completed and submitted to the address listed on the Waiver & Exemption Appeal Form. No further processing of the Hazlewood application will occur, and you will be responsible for any tuition and fee payments, until you meet the requirements or until an appeal is granted.

  • Hazlewood eligible students may also be eligible to receive federal student aid by completing the FAFSA. The exemption is always counted in your financial aid budget. Please note that your awards may be reduced due to the receipt of Hazlewood.
  • Deadlines for submission are viewable on the VA Home Page. UTA will not certify exemptions that are in a past semester. You must submit an application no later than the last class date of the semester to which the exemption applies.
  • Continuing Education Courses and the Executive MBA Program are not eligible for Hazlewood.

Hazlewood for Veterans: (Must reside in Texas while using benefit)

  • DD-214 showing discharge (Member 4 copy)
  • Application form
  • VA letter of exhaustion (NOT ebenefits page) if possibly eligible for 100% Ch. 33 benefits or last used Ch. 31 benefits
  • VA Certificate of Eligibility if veteran served on or after 9/11/2001. COE may be obtained by completing an application on vets.gov
  • Printout of hours used from the Hazlewood Database for Students
  • Active duty orders needed if residing out of state

First Time Legacy Applicants  - Click Here if you are the CHILD of the qualified Veteran

Hazlewood Legacy: (Must be under 26 years old & meet the school's in-state residency requirements) Only one person at a time may use the Veteran's Hazlewood hours.

  • Veteran's DD-214 showing discharge (Member 4 copy)
  • Application form
  • Proof of eligibility, ineligibility or exhaustion, for the student, if any Federal VA Educational Benefits are/were being used (if the veteran served on or after 9/11/2001). Certificate of Eligibility may be obtained by completing an application on vets.gov
  • Proof of dependency (i.e. birth certificate)
  • Both Veteran and Dependent printout of Hazlewood hours used from the Hazlewood Database for Students
  • Active duty orders needed if Veteran is residing out of state

First Time Dependent Applicants - Click Here if you are the CHILD OR SPOUSE of the 100% disabled or service-connected deceased Veteran

*Access to this form is granted once you are admitted & registered for class!

Hazlewood for Dependents & Spouses: (Veteran is 100% permanently & totally disabled or deceased as a result of a service connected illness or injury)

  • Veteran's DD-214 showing discharge (Member 4 copy)
  • Application form
  • Proof of dependency (i.e. birth certificate or marriage license)
  • Proof of eligibility, ineligibility or exhaustion, for the student, of any Federal VA Educational Benefits (if the veteran served on or after 9/11/2001). Certificate of Eligibility may be obtained by completing an application on the vets.gov
  • VA disability rating letter at 100% permanent & total OR DD Form 1300 for deceased veteran
  • Printout of hours used from the Hazlewood Database for Students

This application is the Fall 2019, Spring 2020 and Summer 2020 application for students who have been using Hazlewood continuously at UTA only.  If you have not, please complete the 1st time Application submission process above.

REVOKING HAZLEWOOD

An eligible veteran or designee who has assigned unused Hazlewood Exemption hours to a child through the Legacy Act may revoke the assignment of any unused hours by submitting the Revocation Form
