The documents on this page are all the necessary & related forms and policies in use with UT Arlington Fraternity and Sorority Life. Parts I and II are community wide forms/policies i.e. applicable to ALL UT Arlington Fraternity and Sorority chapters. Parts III through VI are applicable only to the respective council i.e. IFC, PHC, NPHC, or MGC.

Fall 2012 Chapter Minimum Standards
C.E.A.S.R. Chapter Sign-in Sheet (non FSL sponsoredevents)
Roster Change form - to make changes in a member's status (this form is currently closed the deadline for submission was census date)
Roster Addition form - to add members to a chapter roster (all Spring 2013 new members should be added to rosters by March 29, 2013)

II. Student Travel
Only needed if traveling outside of DFW Metroplex. ALL THREE DOCUMENTS MUST BE COMPLETED AND SUBMITTED AT THE SAME TIME. If you have questions on how to fill out these University required documents, please contact the Fraternity and Sorority Life staff at least seven (7) days prior to travel.
