Interpreter/CART Request Form
Important Information Before you Begin
You must be registered with OSD in order to request an interpreter/CART provider. In addition, you must be approved for the accommodation of “Use of a Sign Language interpreter” or “Use of CART services” and have submitted an Accommodation Request Form for the current semester. The Accommodation Request Form is for the classes you are enrolled in for the current semester; however, the Online Interpreting Request Form is to be used for course required lectures or events and meetings with advisors. If you submit a request for interpreter/CART services and you do not have either of these accommodations, or you did not request and pick up your accommodation letters, your request will be denied.
You must complete all information completely and accurately, or your request will not be processed. You may need to refer to your syllabus or your class schedule in order to obtain the information required for the form, so please have that information available.
You must request to schedule an interpreter or CART provider at least 5 days in advance. If the date you are requesting is not within the appropriate time frame, your request will not be processed and you must come to the Office for Students with Disabilities and speak with your OSD counselor in order to make the request.
Once you have submitted your request online you will receive an e-mail with the details of your request. Once a provider has been secured, you will receive an additional e-mail from an OSD staff member confirming your request has been completed.
Requests made after 5:00 PM, Monday through Friday or on the weekend are not considered when figuring the 5 days advance notice, and will not be processed until the following business day.
