Interpreter/CART Request Form
Important Information Before you Begin
You must be registered with OSD in order to request an interpreter/CART provider. In addition, you must be approved for the accommodation of “Use of a Sign Language interpreter” or “Use of CART services. You must have retrieved your accommodation letters, available from your Student Center in MyMav and notified your OSD counselor of your need for Interpreter/CART services for the semester.
The Online Interpreting/CART Request Form is to be used for individual, non-routine events (i.e. study group, meeting with instructor or advisor, etc.). If you submit a request for interpreter/CART services and you do not have either of these accommodations, your request will be denied.
You must request to schedule an interpreter or CART provider three (3) business days in advance. If the date you are requesting is not within the appropriate time frame, your request will not be processed and you must come to the Office for Students with Disabilities and speak with your OSD counselor in order to make the request.
You must complete all information completely and accurately on the Interpreter/CART request form to include begin and end times, or your request will not be processed. You may need to refer to your syllabus or your class schedule in order to obtain the information required for the form, so please have that information available.
Once you have submitted your request online you will receive an e-mail with the details of your request. Once a provider has been secured, you will receive an additional e-mail from an OSD staff member confirming your request has been completed.
Requests made after 5:00 PM, Monday through Friday or on the weekends or holidays are not considered when figuring the 3 days advance notice, and will not be processed until the following business day.If you do not receive an e-mail within two business days of your request, please contact the Office for Students with Disabilities at 817-272-3420 or email@example.com.
