*Note* Alternative formats can take up to two working weeks to process. Submit your requests early in order to have the materials by the time class starts.
Before you request alternative formats, check to see if the material is already available from the vendor (Bookstore, Amazon, etc) in a digital format. Many times electronic formats are less expensive and your material will be available immediately.
Before submitting an Alternative Media Request Form you must be registered with the Office for Students with Disabilities, and have the accommodation of “alternative formats”. If you have not completed this process, you must do so before submitting a request for alternative formats. You must read and be aware of all policies and procedures pertaining to alternative formats.
When making a request, you must complete the form and information for each book you are requesting. You must provide a copy of the receipt showing proof of purchase for each book you are requesting.

If you cannot fill the form out online, you may download a copy to fill out. Once you have completed the downloaded form you may deliver it to the Adaptive Resource Center (ARC), University Hall, room 101, email it to firstname.lastname@example.org or you may fax it to 817-272-3515.
Please note, when possible, the ARC will first submit a request to the publisher for an accessible copy of the book. If the book is not available from the publisher you will be required to submit your copy of the book for production. In addition, you may be asked to provide the ARC with a copy of your class syllabus.
