Telehealth Guide

Please complete the Telehealth Guide steps after you have a scheduled visit. To schedule a visit, call CAPS at 817-272-3671.

BEFORE YOUR ZOOM TELEHEALTH VISIT

Consider technology and privacy requirements to minimize disruptions and maximize confidentiality. A private space is required for all telehealth services to reduce the potential for other people to overhear and to protect your privacy and confidentiality. Please make sure you have the Zoom app downloaded to your device and a free Zoom account established.

AFTER SCHEDULING FIRST-TIME VISIT OR AS INSTRUCTED: BEGIN PAPERWORK VIA WEB PORTAL

Complete A 1 Patient Forms Packet and B 1 CAPS Consent, Telehealth, and Emergency Forms. Ensure forms are fully completed and submitted. You may then skip Step 2 in step-by-step guide below.

30 MINUTES PRIOR TO SCHEDULED VISIT: COMPLETE PAPERWORK VIA WEB PORTAL 

Login 30 minutes prior to appointment to complete questionnaire (SDS) and survey forms (CCPAS-62). All paperwork must be completed (Steps 1-4 below) prior to start of visit.

If Non-Crisis/Triage Visit: Ensure Forms A 1 and B 1 (Patient Forms Packet + CAPS Consent, Telehealth, and Emergency) are complete.  
If Crisis/Triage Visit: Complete Forms B 1 + B 2 (CAPS Consent, Telehealth, and Emergency + CAPS Triage Communication Preference).  

Web Portal

STEP-BY-STEP GUIDE:

  1. Log into web portal (https://mavcare.uta.edu) with UTA login and password. Confirm your date of birth and "Proceed".
  2. Left-hand side: FORMS. If First-Time Telehealth Visit or Instructed: Complete FORMS A 1 + B 1. If Crisis Visit: Complete FORMS B 1 + B 2.
  3. Left-hand side: APPOINTMENTS ->Appointment Check In. Confirm your physical location (must be in Texas unless in crisis).
  4. Click "OK" next to “Check-In to Online Appointment”- > DO NOT CLICK: If given “Click here to join the meeting” option.
  5. Click “Return to Appointments”.
  6. Click on “Complete Questionnaire”. Complete all applicable questions, click “Submit Final”.
  7. Left-hand side: SURVEY FORMS. Select CCAPS-62 (unless CCAPS-34 instructed). Complete all questions, click “Finish”.
  8. Left-hand side: APPOINTMENTS. Select "Join Meeting" to enter the Zoom waiting room. Please wait here until the counselor starts the session.
