As a patient of UTA Health Services, you have the right:
- To be treated with respect, consideration, and dignity.
- To be provided appropriate privacy at check-in and while being seen.
- To know your provider’s name and credentials.
- To be given complete information regarding your diagnosis, treatment, and prognosis when medically advisable and known. Such information may also be provided to another person under the following conditions: during a medical emergency, when it is medically inadvisable for you to receive the information about your prognosis, when the person has legal authorization, or when you give authorization for the person to receive the information.
- To be given the opportunity to participate in decisions involving your health care.
- To change providers if other qualified providers are available.
- To decline evaluation and/or treatment by a health care professional in training.
- To be informed of the right to refuse to participate in any research at UTA Health Services.
