This form should be used by students who have submitted an official degree plan signed by their academic advisor and a VA certificate of eligibility to the Veteran Benefits Certification Office.
We require all students to certify online EACH semester at UTA. List all classes you are officially enrolled in and would like the VA to provide reimbursement for. If you are not officially enrolled in a class, it WILL NOT be certified. If you are on the waitlist for a class, it WILL NOT be certified.
If you drop a class you are required to certify online for each drop. If applicable, mitigating circumstances must be submitted to email@example.com.
Please verify that your student ID# is entered correctly.
You are responsible for submitting the correct chapter and eligibility % for the semester in which you are certifying. The U.S. Department of Veterans Affairs determines eligibility, not UTA. If you have not received a Certificate of Eligibility or are unsure of your benefit type contact the VA prior to submitting this form.
