FERPA Form

FERPA (The Family Educational Rights and Privacy Act) protects the privacy of your education records. By completing a FERPA delegation, you can authorize the Office of the Dean of Students to discuss specific information from your student record with a person you choose, such as a parent, guardian, spouse, or other trusted contact. This authorization helps ensure that information is only shared with individuals you have approved and in accordance with federal privacy regulations. You may update or revoke your delegation at any time.

Follow the steps below to add or manage your FERPA-delegated contact.

  1. Login to MyMav with your student credentials
  2. Scroll down to “Share My Information Tile” and select
  3. Select “Delegate Access to a New Contact”
  4. Review the Terms and Conditions
  5. Complete the form
    • To delegate access to information for the Office of the Dean of Students, select “Student Conduct and Outreach”
