Benefit News

2017-2018 Insurance Plans

Employees can expect no rate increases in 2017-18 for the UT Select medical plan and voluntary benefits such as the dental, vision, life and accidental death insurance, and long- and short-term disability plans. If you have any questions, contact the Benefit Services Department at benefits@uta.edu or 817-272-5554.

Benefits Fair

The Benefits Fair is on Tuesday, July 18, 2017 from 9:00 am to noon at the Maverick Activities Center (MAC) in the lounge. Insurance and retirement vendors will be present. Door prizes will be given out. The Benefit Staff will be available to answer any questions you may have about the upcoming benefit year. A shuttle bus to the MAC will be continuously running during the Benefits Fair from LOT 1 (HR parking lot) to the MAC. If you have any questions, email benefits@uta.edu or call (817) 272-5554.

Health Checkups

Onsite health checkups will be administered by Catapult Health on Tuesday, July 18, 2017 from 9:00 am to 3:00 pm at the MAC. Click here to schedule an appointment. Each participant receives a comprehensive personal health report and a one-on-one virtual consult with a nurse practitioner. This event is open to UT Select medical plan members. It is free. Affordable Care Act (ACA) Information

Group Benefits Handbook, Plan Guides , Retirement and All Forms

Required Documentation for Eligible Dependents. Documentation is now required for all dependents when adding coverage as a new employees, family status change or annual enrollment.  Documentation will be required even if the dependent has current coverage on another plan.

IRS Form 1095-C

Employer-Provided Health Insurance Offer and Coverage 

As part of the Patient Protection and Affordable Care Act, also known as Obamacare, employers are required to send Form 1095-C to certain employees.  It will contain detailed information about your health care coverage. In February 2017, you may receive this form.

You will receive the form if you meet either of the two conditions listed below.  

  • Were enrolled in health care coverage from UT Arlington at any point in 2016
  • Worked an average of 30 or more hours per week 

It is important to keep the form for your records since you will need it to file your taxes for 2016.  You can view the blank form here on the IRS website.
