SURGE Program Application

To be eligible for admission into the SURGE program and for the S-STEM scholarship, a student must

• be a United States citizen, national, or permanent resident at the time of application
• demonstrate financial need through the FAFSA application
• be pursuing a bachelor's degree in Mathematics at UT Arlington
• be enrolled full-time (12 or more credit hours) during the semester receiving the scholarship
• have a GPA of 3.0/4.0 or higher

Applications are accepted any time. The start of the SURGE scholarship for an applicant may be affected by the application. Interested students should submit the following via e-mail in paper format to any one of the SURGE faculty:

• Student ID number (1000 ID number)
• Student Aid Report (SAR) to verify financial eligibility. You may obtain this through FAFSA (Free Application for Federal Student Aid).
• Unofficial transcripts in a readable format (e.g. PDF)
• Resume if available
• A brief statement of current academic standing, interest in mathematics, plans to graduate, and career plans after graduation
