- Every applicant must submit a completed form DS-11 in black ink. You can either complete this online and bring it with you to your appointment or complete the form in the UTA Passport Office. All DS-11 forms must be printed single-sided. DO NOT SIGN THE DS-11 UNTIL YOU MEET WITH THE PASSPORT ACCEPTANCE AGENT IN PERSON.
- Proof of US Citizenship - You must provide one of the following items to the UTA Passport Office when apply for a US Passport:
- Original or Certified Birth Certificate
- Must be issued by the city, county and state of your birth.
- Must be an original or certified copy. Photocopies are not acceptable.
- Hospital certificates are not accepted.
- Certified copies of Texas birth certificates may be obtained from any county clerk’s office in the state.
- Birth certificates must included the full name, date of birth, place of birth, and the full name of both parents.
- The seal of the issuing authority and official signature must be visible.
- Certificate of Naturalization
- Certificate of Citizenship
- Consular Report of Birth Abroad
- Undamaged US Passport Book or Card
- Check or money order for fees paid to US Department of State
- Debit or credit card for photo and execution fee
- Valid State Issued ID such as a driver’s license or identification card. Provisional, temporary, and expired licenses, as well as social security cards, do not fulfill this requirement.
- If your State Issued ID is from a state other than Texas, please bring a secondary form of ID with your photo (student ID, for example)
- One passport photo (available onsite for a fee)
Notes:
*Regarding Texas Abstract birth certificates: If you have an abstract of your Texas birth certificate, it must include a capital letter “I” after the date filed to be acceptable as proof of US citizenship.
*Abstract birth certificates from other states will not be accepted.
