In alignment with federal requirements* outlined in the 2025 Paperwork Reduction Act, The University of Texas at Arlington now provides Form 1095-C electronically for eligible employees, former employees, retirees, and other covered individuals.
Beginning with the 2025 tax year, paper copies of the form will be available only upon request.
If you are actively employed by UTA and would like an electronic copy of your 1095-C, you can access your form using the options below. Download the instruction guide for step-by-step instructions.

If you are retired or no longer employed by UTA, you can download an electronic copy of your Form 1095-C through our external portal, PaperlessEmployee.com/uta.
You must authenticate your identity by creating an account and logging in with a username and password. Review Option 2 of the instruction guide below:
Form 1095-C is issued only for months in which an individual was enrolled in the UT SELECT Medical Plan, which is the UTA's reportable self-insured health plan. If you were enrolled in UT SELECT for part of the year and UT CARE for part of the year, you may receive a Form 1095-C. The form will reflect only the months you are enrolled in UT SELECT. Enrollees in the UT CARE Medicare Advantage plan will not receive a 1095 form from Blue Cross Blue Shield or UT. Medicare will send you a Form 1095. If you need a replacement form, please contact Medicare at 1-800-MEDICARE (1-800-633-4227).Why You May or May Not Receive a Form 1095-C
