Your annual 1095-C form will be available online after January 25 each year.

In alignment with federal requirements* outlined in the 2025 Paperwork Reduction Act, The University of Texas at Arlington now provides Form 1095-C electronically for eligible employees, former employees, retirees, and other covered individuals.
Beginning with the 2025 tax year, paper copies of the form will be available only upon request.
If you are actively employed by UTA and would like an electronic copy of your 1095-C, you can access your form using the options below:

If you are retired or no longer employed by UTA, you can request an electronic copy of your Form 1095-C through our external portal, PaperlessEmployee.com/uta.
You must authenticate your identity by logging in with a username and password. You can then download an electronic copy of your 1095-C.

If you no longer have access credentials or are having issues logging in, please contact askhr@uta.edu for assistance on retrieving your form.

*These requirements do not apply to current, former and retired employees living in California, New Jersey, Rhode Island or Washington, DC. Employees in these locations will receive a paper Form 1095-C from UT Arlington by mail unless they have opted in for electronic delivery.
