- To receive complete, current information regarding your diagnosis, evaluation, treatment, and prognosis.

- To decline participation in any research conducted at UTA Health Services.
- To be notified of any time-relevant COVID-19 exposure within the facility in accordance with state and local requirements and COVID-related policies and procedures.
