Purpose of this request form:

• Secondary and All-Level students request to have their Academic Department holds removed
• Request to be added to the Education courses permission lists 

BEFORE COMPLETING THIS FORM, PLEASE HAVE YOUR DEGREE PLAN IN FRONT OF YOU. SELECT ONLY COURSES YOU NEED. SELECTING ALL, OR THE WRONG ONES, WILL DELAY YOUR CLEARANCE!

Your registration date will be listed in your Student Center (know that not all students register on the same date). It is based on the amount of hours you have accumulated. Note: Advisors DO NOT establish, nor can they change, this date.
